The provider was managing rapid waitlist growth with the same team headcount they had two years ago. A regulatory change in November 2025 was about to make their intake, reporting, and care-plan documentation requirements significantly more onerous. They had bought AI tools the year before. None of them had been adopted.

Identify
Two-week diagnostic across care coordination, intake, and reporting. We interviewed 11 staff across three regions and shadowed one full intake cycle. The bottleneck was clear: 60% of coordinator time was spent on documentation and 1:1 stakeholder updates.
Build
Four workflows, scoped to ship inside their existing tools. (1) Intake document extraction + CRM auto-population, (2) Care plan first-draft generation from clinician notes, (3) Stakeholder update auto-summarization, (4) Funding compliance checklist generation for the new reform.
Adopt
Three weeks of hands-on rollout, one region at a time. Coordinators trained in 90-minute sessions. We embedded with two senior coordinators for daily Slack-based support during week one of each rollout.
